Description Abstract The successfulness of advanced pentavalent immunization program is determined by its coverage. The mothers’ less knowledge of it is a factor to its low coverage. Thus, the extension on the immunization can be used as an effort of improving the mothers’ knowledge of such advanced pentavalent immunization. The aims to investigate the effect of extension on the mothers’ knowledge of advanced pentavalent immunization of the toddlers under three years of age in Keprabon Ward, Surakarta. This research used the quasi experimental method with the non randomized control group pretest posttest design. The samples of research were the mothers whose toddlers were under three-years old. They consisted of 64 respondents and were taken by using the total sampling technique. The data of research were collected through questionnaire and analyzed by using the Mann-Whitney’s Test. The result of research shows that the p-value based on the Mann-Whitney’s Test was 0.000. The research concluded that there was an effect of extension on the mothers’ knowledge of advanced pentavalent immunization of the toddlers less under three years of age in Keprabon Ward, Surakarta.  Keywords: Advanced pentavalent immunization, knowledge, extension
