Introduction: Porphyromonas gingivalisunderlies the relationship betweenperiodontitis with atherosclerosis because has pathogenecities factor, so it is able to invade host cells. The most important stage is defensestage by lipopolysaccharide (LPS) of P. gingivalis.Toothpastewith SLS that can not penetrate on LPS of P. gingivalis. However, siwak (Salvadora persica) containsisothiocyanate that is able to penetrate the LPS of P.gingivalis. This research is intended to prove the effect of brushing teeth with toothpaste and siwak towards P.gingivalis with RT-PCR method (Real Time-PCR).Method: This research was laboratory experimental with pre and post test design, used 36 respondents (men and women) divided into 2 which are brushing teeth with toothpaste and the combination of toothpaste and siwaktowards P.gingivalis during 10 days. The sample on saliva and gingiva were taken before and after treatment. DNA of P. gingivalis was quantificated with quantification cycle value (cq value) used 40 times to acumulate the fluoroscent through threshold.Data of before and after treatment was analyzed with Wilcoxon test and differences of two variables with Mann-Whitney testwith the significance level P<0.05.Result: dsDNA of P.gingivalis on saliva before brushing with toothpaste was detected on 25,86 cycles and after the treatment on 30,09 cycles (P>0,05), on gingiva, before the treatment dsDNA was detected on 25,73 cycles and after the treatment on 32,08 cycles (P<0,05). dsDNA of P. gingivalis on saliva before brushing with the combination of toothpaste and siwakwas detected on26,16 cycles and after the treatment on 33,62 cycles (P<0,05), on gingiva,before the treatment dsDNAwas detected on 25,94 cycles and after the treatment on 32,57 cycles (P<0,05).Difference dsDNA of P. gingivalis before and after treatment in saliva group 1 was 4.23 and group 2 was 7.46 (P> 0.05). Difference dsDNA of P. gingivalis before and after treatment in group 1 gingiva was 6.35 and group 2 was 6.62 (P> 0.05).Conclusion: The brushing with toothpaste shows P.gingivalis on gingivadecreased significantly but on saliva decreased insignificantly. The brushing with the combination of toothpaste and siwak showsP.gingivalis on saliva and gingiva decreased significantly. Brushing with toothpaste and the combination of toothpaste and siwak are different but not significant both in saliva and gingiva.Keywords: siwak,toothpaste, Porphyromonas gingivalis, RT-PCR
